2. Complete the Interested Player Form below to receive correspondence from the CHS Boys Soccer Board about our upcoming 2020-2021 season.

3. Fill out these forms! YOU MAY NOT TRY OUT WITHOUT THESE FORMS AND YOU WILL NEED A PHYSICAL TO FILL THEM OUT.

All Cambridge High School sports will be utilizing the DRAGONFLY health management system to manage our athletic participation forms process in 2020-2021. Please use the link below to find out how to register your player with the Dragonfly system to complete all health and participation forms necessary for Cambridge sports.

These forms must be completed, submitted and approved by the CHS athletic trainers prior to athlete participation in any preseason and seasonal soccer workouts and training. NO EXCEPTIONS
